1. Fill out all the information on your order Form. 

2. Enter the items you wish to purchase (along with price).
 

3. Total the prices of the items you wish to buy, add *shipping and handling if any.

4. Print the order form: (Credit Cards users) Sign your name after printing for authorization)

For your convenience please double check your credit card numbers to make sure they are correct. Otherwise there may be a delay with your order.

5. Credit card users make sure you put the billing address and zip code of your credit card on the order form.

6. If you wish to send your order to another person please write that persons name on the order form after printing.

7. Credit Card users, If your order is a gift and you don't wish to have your receipt attached to your order, please write this on your order form.

8. Write any special instructions you have about your order on the order form.

9. For +overnight delivery (USA only) there will be a $20.00 charge for shipping and handling, unless otherwise specified, the default will no have a signature when order is received.

  Most orders are shipped within *5 business days.

  We make all eye patches one at a time and inspect each patch before shipping.

The internet has several security problems concerning identity theft and stolen passwords to Pay Pal and other online pay services. Because of this we prefer to do business by checks, money orders, Visa, or MasterCard (Sign printed order form)

+If your order is received before 4:00pm, overnight delivery is mailed after 24 hours after eye patch order is received. If order is received after 4:00 pm, please allow 48 hours before order is mailed.

Eye Patch orders by check are sent as soon as the check has cleared the bank

**Sizes are approximate. For the best position the band should be worn approximately 1/4” to one 3/4" above your opposing eyebrow and arch gently over the brow. The portion of the band on the side of the patched eye should be worn slightly above your ear.
