Choose from 2 kinds of forms Word (fill in online then print) or PDF form (print then fill out the form) and mail or fax it along with your Credit Card information to 1-918-343-2718, or mail your order with your Personal Check, Cashiers Check, or Money Order to Eye-GrafX, P.O. Box 2433, Claremore, Ok 74018. Please note, we reserve the right to delay shipment up to two weeks for orders paid via cashier checks, and money orders for verification purposes.

Processing Time:

We strive to process your order within 5 business days (Monday-Friday) of receiving both the order and payment (Personal Check, Cashiers Check, and Money Order (via mail). Please note, we reserve the right to delay shipment up to two weeks for orders paid via cashier checks, and money orders, or Personal/Business checks for verification purposes) or after approval .
