(Make sure you write as legible as possible) For your convenience please double check your credit card numbers to make sure they are correct. Otherwise there may be a delay with your order.

5. Credit card holders make sure you put the billing address and zip code on the order form.

6. Write any special instructions you have about your order on the order form.

7. For overnight delivery there will be a $20.00 charge for shipping and handling.

The internet has several security problems concerning identity theft and stolen passwords to Pay Pal and other cards online. Because of this I prefer to do business by checks, or credit cards (filled out after printing the order form) mailed to P.O. Box 2433 or faxed to 1-918-343-2718.  Some people have sent the order to me via email as a jpg or tiff file if I have your credit card on file.
